Below is a list of required documents needed for registration:
- Verification of Legal Name
Official Birth Certificate or Passport

- Immunizations and Physical Exam
If you do not have a doctor you may find assistance on our Resource list of Medical Providers page.
Proof of Immunization on a Form 680 obtained by your doctor or Orange County Health Department:
- Verification of Academic History
Transcript, withdrawal form or last report card.
- Verification of Special Education Information (If applicable)
Current IEP or 504 Plan
- Verification of Residence
Home Ownership- Current Homestead Exemption Card, Current property tax statement, closing contract or warranty deed, along with current utility bill, ID with current address and current mail.
Renting or leasing- Current signed lease, ID with current address, current utility bill, rent payment/statement, current mail and renters insurance.
Living with someone else- You will need to obtain a Verification of Residence form from the Student Enrollment Office. Please visit OCPS.net to find what documents will be needed and make an appointment with their office to obtain form.
- Verification of Guardianship
Birth Certificate, Court Custody Documentation (if applicable), Divorce Decree(if applicable), Parenting Plan (if applicable)
If your child is living with someone else and there is no legal documents please see the office of Student Enrollment for OCPS Educational Guardianship.
